Registration for Baseline Testing

Thank you for your interest in A Step aHead Baseline ImPACT® Testing.  By participating in this A Step aHead initiative, you are taking an active step in brain health and safety in youth sports.  Please fill out the following online registration form.  When you have successfully completed your registration, a confirmation email will be sent to the email address you provide.  Within 48 hours of your registration, you will be contacted by an AthletiCo Physical Therapy clinic to schedule the date and time of your ImPACT test.
