Screening Form Confirmation

Thank you for your submission. Response and a final decision may take up to 5 business days. If this is an urgent request, please reach out to your program manager to expedite the process.

PLEASE KEEP THE CONFIRMATION EMAIL YOU RECEIVE

Follow the action steps below within one week of completing the event:

1. Visit the Clinical Program Outreach Recap page and fill out the form
   Tip: visit the form prior to your event to know what you will need to submit after the event!
2. Follow-up with screen participants that require further communication, and be sure to thank them for participating
3. Thank the organization for the opportunity to provide screens – leave a positive lasting impression, and don’t forget to ask them for future opportunities.
4. Submit any photos from the event to socialmedia@athletico.com. A waiver form will need to be signed by anyone in the photos, other than employees. Minors will need parental consent. The waiver form can be found here
5. To find these costs please visit the marketing ordering system, login, and select the materials you will use at the event. Unsure of what to bring? Please reference the document “How to use Athletico Branded Marketing Materials” located on sharepoint, here.
